On the AAUP Sick Leave Donation Request Form, the form states that a "treating physician's written documentation showing the need for the leave following the exhaustion of my accumulated sick leave."
That statement was added to address the member's need for as much as 20 days leave (or the additional 20 days). It does not apply to members who run out of sick time and need to use sick leave for one to five days (approximately).
If a member is sick and they do not have any sick time remaining, they should call in sick using the leave reporting system, and then submit the above form to Kerry Gilbreth and Karen Kraus in HR.
